We have an exciting opportunity for a qualified nurse to join our busy Older Adult Mental Health Team Oxford City and North East, which is responsible for providing care and treatment to Older Adults with mental health problems in the community.

We are looking for a positive, reliable and flexible individual to join the team as a Community Mental Health Practitioner.  Our team offers support 7 days a week 9-5pm and occasionally 1-8pm

You will be part of a motivated, supportive and cohesive multi-disciplinary Mental Health Team where your contribution is highly important. This role is open to registered professionals with a registered mental health nursing qualification. The team actively promotes a multi-disciplinary approach to the care coordination of our clients.

The post holder will work within the Team providing:

  • Mental health assessment and treatment in the community for clients with mental health needs.
  • Supporting clients, families and informal carers through mental health crises range of specialist interventions.
  • Supporting clients to work towards their optimum level of recovery.

The post holder will:

  • Act as Care Coordinator under the framework of the Care Programme Approach for a caseload of clients.
  • Provide timely access to assessment of mental state and psychosocial intervention for clients experiencing a mental health crisis.
  • Work under the direction and supervision of a Deputy team Manager as part of a multi-disciplinary team.

We are looking for a full time registered mental health nurse who:

  • Is an enthusiastic and resourceful person with the ability to work within a team, independently and with initiative.
  • Has experience of working in adult mental health services
  • Is familiar with current legislation, guidelines and processes that influence the way in which mental health care is delivered.
  • Has a good understanding of mental illness and its impact on service user’s 
  • Has excellent skills in assessing and managing risk and Safety Planning 
  • Has good communication skills in terms of telephone and face to face contact 
  • Is interested in driving forward quality nursing leadership
  • Will carry out the role of nurse mentor/supervisor to students 
  • Has a valid driving license and use of a car for work.
