Oxford Health NHS Foundation Trust has been rated as Good by the Care Quality Commission (CQC).
We are a responsive and innovative Foundation Trust that places our values of Caring, Safe and Excellent at the heart of everything we do. We take great pride in our staff and believe that investing in you is key to us achieving our vision of outstanding care delivered by outstanding people.
We work with a wide range of partner organisations to deliver care and support to people in their own homes and from a number of hospitals and community bases. We focus on delivering care as close to home as possible.
We are looking for experienced Mental Health Nurses to join our Assessment Team. Based in Banbury and covering North Oxfordshire, you will be responsible for undertaking assessments of people referred for secondary mental health care by local GPs. All assessments are undertaken with another clinician, and treatment plans discussed with the MDT where appropriate.
The role includes shift work, providing step up care for individuals experiencing a mental health crisis, at risk of admission or step down post discharge. Out of hours, this work is undertaken with our colleagues in Witney, and covering North and West Oxfordshire, so a car driver is essential. We use the FACT model to prioritise work and share information with our Treatment Team and Day Hospital colleagues.
You will be experienced in undertaking mental health assessments, supporting people with mental health problems in the community and managing a caseload. Experience of delivering brief interventions is desirable, although we have lots of resources to share and use.
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
